Situations:

1.      Drug and violence related incident for a Regular Ed and Special Ed with at least 1 day suspension.

a.      Incident (entry must have the DOE acronym)

b.     Offense Indicator= Y

c.     Discipline Indicator = Y

d.     Consequence 1: (entry must have the DOE acronym)

 

(Always report in SSDR)

2.      Special Needs student suspended 1 day or more.

a.      Offense Indicator = N

b.     Discipline Indicator = Y

c.     Consequence 1: (entry must have the DOE acronym)

 

(Always report in SSDR)

3.      Not a drug or violence related incident for a Regular Ed student suspended for 11 days or more.

a.      Offense Indicator = N

b.     Discipline Indicator = Y

c.     Consequence 1: (entry must have the DOE acronym)

 

(Never report in SSDR)

4.      Not a drug or violence related incident for a Regular Ed student suspended for 10 or less days.

Alternate Education Code:

            Use this field to indicate if Alternative Education was provided.

§         Select N for No

 

Program Status Code:

The program code is an indication of the student’s regular or special education status.

§         Select the Program Code

1.      Regular Education

2.      Special Education – student has a chapter 766 individualized education plan

 

Alternative Education Type:

The Alternative Education Type indicates the type of Alternative Education that was offered in reference to the Discipline Record.

Bodily Injury Incident (SPED STUDENT ONLY):

                        If student is NOT SPED

§         Leave blank

 

If student IS SPED

§         Select N for No and Y for Yes

 

Alternative Education not provided:

The Alternative Education not provided indicates why the Alternative Education was not provided.
